Please make XML Doc Comment classes public

This includes ReferencePathUtilities and either FileBasedXmlDocumentationProvider or IDocumentationProviderService.

These are necessary when working with XML doc comments, especially for editor scenarios.

Please also provide a public way to create VisualStudioDocumentationProvider for VS editors bound to custom workspaces.

It turns out that I can create VisualStudioDocumentationProvider using VisualStudioWorkspace.CreatePortableExecutableReference.

I still want public methods to create XML doc providers for non-VS scenarios, and I still need ReferencePathUtilities in both cases.
